Hazard Log

Every Hazard with its severity and likelihood before and after its controls, the controls that mitigate it and whether each is verified, its owner and its acceptance. The page is part of Safety & Reliability: turn on Project Preferences → Program features → Safety & Reliability (on automatically with the DoD or NASA process guide).

Where to find it

  • In the navigation rail: Safety & Reliability → Hazard Log.

Main actions

  • Top bar: Hazard Matrix… (the risk level of each likelihood and severity pair; MIL-STD-882E levels High, Serious, Medium and Low by default), Export ▾ (Excel, CSV) and + New Hazard (name and a suggested number such as HAZ-002).
  • The grid at the top counts hazards by likelihood and severity, Before controls or After controls (residual values; hazards without them count at their initial values). Click a cell to list only its hazards.
  • Columns: ID, Hazard, Status, Severity, Likelihood, Risk, Causes (caused by: assets, functions, items, failure modes), Controls (mitigated by: requirements, functions, assets, documents), Verified, residual Severity, Likelihood and Risk, Owner, Program Risks (causes a Risk) and Accepted. Status, severity, likelihood and owner are editable in place.
  • Verified counts controls whose verification passed out of those that can be verified: a requirement by its RVTM status, a test case by its result. A design feature or procedure is verified through a requirement; point at the cell for each control.
  • Needs attention (the ! beside a name, and the filter): no control on an open hazard, Controls Verified or Closed with a control not verified, a failed control, a High or Serious residual risk without Accepted By, or Accepted without an acceptance record.
  • Click a row to edit the hazard in the inspector (Esc deselects). Relate causes, controls and program risks in its Trace section.

Tips

  • Write the controls as requirements and verify them with test cases: the Hazard Log then follows the RVTM.
  • Set Eliminated likelihood when a design change removes the hazard; it shows as its own level.
