Safety & Reliability

FMECA

Failure modes, effects and criticality analysis: each Failure Mode of an asset or function with its cause, its effects, its rating and the controls that compensate for it. Part of Safety & Reliability (Project Preferences → Program features).

Where to find it

  • In the navigation rail: Safety & Reliability → FMECA.

Main actions

  • Top bar: Method (RPN: Severity × Occurrence × Detection, each 1 to 10; Criticality: Severity Class I–IV, Probability Level A–E and the mode criticality Cm = β · α · λp · t), Export ▾ (Excel, CSV) and + New Failure Mode (choose the asset or function first; the failure mode is caused by it, one undo step).
  • The worksheet groups failure modes by their item, in number order; with the criticality method each item header shows its criticality Cr per severity class. Columns: ID, Failure Mode, Cause, Local Effect, End Effect, the ratings, Detection Method, Controls, Verified, Hazards & Risks (what the failure mode causes) and Status.
  • Severity, Occurrence and Detection accept whole numbers from 1 to 10. RPN at or above the limit (100 by default; change it beside the filter) is shown in red; the RPN ≥ filter (or Severity class I–II) lists only those.
  • Enter Failure Rate (per million hours), Failure Mode Ratio (α), Failure Effect Probability (β) and Operating Time (hours) in the inspector to get Cm. Long texts such as the effects and the recommended action are edited there too.
  • Relate a failure mode to the Hazard it can lead to (causes): the hazard lists it as a cause.

Tips

  • The method and the RPN limit are project settings; changing them is one undo step.
  • Rate consistently: write down what 1 and 10 mean for your project.
